We will need you to fill out a few forms before your visit. This will help Dr. Chene learn about you and your health history beforehand, allowing her to make the most of your time together. You can download these forms off this website (in the Downloads section) or when you contact our office for an appointment, we can mail the forms to you. Feel free to add more details as you answer the questions. Don't forget to write the approximate dates of when each problem occurred.   If you forget your papers on the day of your visit, please come 30 minutes early to fill out the forms. For your convenience, a map with directions to the Healing Arts Center is on this website and in the booklet you receive when you make an appointment for Personalized Prevention Programs.

Results for most lab work are available within 2 days, but many tests must be sent to a specialty lab in another state. These results may not be available for 2 weeks or more. We will notify you at the time of your follow-up appointment, which is generally within 4 weeks of your lab work. If the results are urgent we will notify you by phone or mail. If all labs are normal and you are not needing a follow-up appointment, we will notify you of all cancer screening results (ie: mammogram, Pap Test, PSA etc.) If you do not receive these results within 4 weeks after your test, please call Dr. Chene's office.

Dr. Chene has selected certain high-quality supplements to make available in her office for patient convenience, quality issues, and, in most cases, lower prices. This is particularly true if the supplements are difficult to find or if there are concerns about quality or price. Supplements are currently only available by phoning the office but will soon be available directly from this website.
